Fraudulent Scripts
8-17-2024: Be aware of a fraudulent written prescription brought in (Mineral Wells) for Bactrim DS and Promethazine with Codeine from Dr. Finbar Powderly, MD for patient last name Frazier DOB 10/12/61). The prescriptions are confirmed fraudulent.
8/16/2024 fraudulent prescription phoned inunder the provider Robert Wynn. Calling in phenergan with codeine for patient with DOB 12/18/1968 and last name Preston. The prescription has been confirmed with provider to be fraudulent. Please be aware.
5/28/2024: Pharmacies report a phoned in prescription for a Z-Pak and Promethazine syrup. They provided proper name, NPI, 304-726-4860, Jeffrey T. Cook from One Medical Choice in Keyser, WV. Similar call to another pharmacy under provider John Cook. This is almost identical to some from about 10 days ago. Please be alert.
5/18/2024: Be aware of confirmed fraudulent prescription phoned in 5/17/2024 under the provider Robert E. Wynn, Jr. (correct NPI and DEA), wrong phone.) Calling in for patient with DOB 2/18/1997 and last name Preston for promethazine with codeine #200ml with a refill. The prescription has been confirmed with provider to be fraudulent. Please be aware.
5/15/2024: Pharmacies report a phone call from a private caller ID. This number proceeded to call in a prescription for a Z-Pak and Promethazine syrup. They provided proper name, NPI, phone number, and address for a provider, Jeffrey T. Cook. The pharmacist proceeded to attempt to contact the provided number which led to an out of service message, and all other numbers on the provider's profile in our system were for different hospitals. The pharmacist was unable to contact the provider and another local store received a similar prescription combo last week. Please be alert.
10-30-23: Fraudulent scripts that were telephoned in to the Morgantown, WV area. The caller claimed to be from Dr. Selwyn Adams office in Fredericksburg, Virginia and was trying to call in a Zpak and Phenergan with Codeine. The caller correctly gave the doctor’s NPI #, DEA, address, but the phone number (a cell number -703-672-7852) and the spelling of the first name was incorrect. When the pharmacist called the office 540-374-1188, they were able to confirm it was not them. The patient name that the caller gave was “Adriana Stewart” with a DOB of 08/11/1989
9-18-23: Dr. Carl Stephen High, Elkins Allergy Center has confirmed fraudulent prescriptions being phoned in at multiple pharmacies for promethazine and amoxil. This is sent out at his request. Please confirm any suspicious prescriptions with his office at 304-636-9242. Please be on the lookout for these forged prescriptions to prevent diversion.
9-1-23: This similar situation has been reported a couple of times in southern WV so we wanted you to be aware of it. A "doctor" calls in prescriptions but the voice sounds odd like a robot or machine. Caller ID was Glen Dale, MD. Individual identified himself as Dr. Kevin Williams calling in both alprazolam and promethazine with codeine for a patient who was not in the pharmacy system. Last name of the patient was Davis. These were not valid prescription orders. Please be aware.
8/17/23: Fraudulent prescriptions received in Huntington, WV area from Micheal Schnyder, PA out of Seattle Washington. The DEA used was MS2503527. The PA has a different DEA. This was for promethazine with codeine plus, ibuprofen and a zpak.
7/19/2023: Fraudulent prescriptions are being phoned in by someone in the name of orhopedist Dr. Joseph Fazalore og Bridgeport, WV. They do have his correct DEA number. Most prescriptions from him will be e-scripts. Any questionable prescriptions can be verified at his office at 681-342-3508.
